HOW WE SAVE YOU MONEY

As a non-insurance program, members need only present their card in order to receive the savings without worrying about claim forms, long waiting periods, age restrictions or medical conditions.

That's it! The Formulated Dental Plan is not insurance, so you don't have to worry about paperwork. Discounts are received immediately at time services are rendered. The minimum term for Formulated Dental Plan Membership is one (1) year. Membership fees are drafted monthly. We accept credit cards or electronic checking account drafts only.
